HOW TO MEASURE YOURSELF

Weigh and measure yourself in fitted clothing or in the buff for the most accurate results. Use a measuring tape to measure your waist and hip circumference.
- For your neck, place the measuring tape where the top button of a button-up shirt would be and measure parallel around your neck.
- For your chest, place the measuring tape at the largest part of you chest and measure all the way around keeping the tape parallel with the floor.
- For your upper arm, measure the circumference about ½ inch below your armpit.
- For your waist, place the measuring tape about ½ inch above your belly button and measure parallel around your torso.
- For your lower abs, place the measuring tape about ½ inch below your belly button and measure parallel around your torso.
- For your hips, place the measuring tape across the widest part of your hips/glutes and measure all the way around, keeping the tape parallel with the floor.
- For your Upper thigh, sit on a chair and place the measuring tape about ½ inch from your groin/hip and measure the circumference.
- For your mid thigh, sit on a chair and place the measuring tape exactly half way between your knee and your hip.
- For your calf, place the measuring tape at the widest part of your calf and measure the circumference.

HOW TO CALCULATE BODY FAT

Any of the following options will be accepted for the body fat measurements:

- BIA scale (Image required: contestant on the scale displaying body fat percentage with a date stamped newspaper)
- Skin fold fat caliper (Image required: skin fold fat caliper on contestant
- Hydrostatic Weighing (Image required: Date stamped certified results)
- Body Composition Test (Image required: Date stamped certified results)
- Most local gyms, health facilities, and pharmacies can provide assistance on your body fat calculations.

HOW TO TAKE YOUR BEFORE & AFTER PHOTOS

Have a friend or family member use a high-resolution camera to take your photo in fitted clothing with solid colors, no patterns and no artwork. Take photos in a well-lit area with a solid background and make sure your entire body fits in the frame. Take both a front and side picture with a date stamped newspaper.
- Participants must submit all photos (holding a date stamped newspaper) and measurements by March 31, 2019 at 11:59 PM (MDT). The Contest evaluation will be based on the difference between your earliest and last photos that were taken during the Contest Period.
- Women must wear a sports bra and fitted shorts.
- Men must wear fitted shorts, no shirt.
- All photos become the property of the Contest Sponsor.
- Any missing measurements, images, or information from participants will result in disqualification.

Who Can Enter?

This Contest is sponsored by LifeVantage Corporation (the "Contest Sponsor" or "LifeVantage") located at 9785 South Monroe Street, Suite 400, Sandy, Utah 84070.
Any LifeVantage customer who lives in the US or Canada (excluding residents of the province of Quebec) may enter the LifeVantage PhysIQ Body Transformation (the "Contest"). Winners of previous LifeVantage PhysIQ Body Transformation challenges are eligible to participate.
The PhysIQ Body Transformation can only be participated in by participants that are 18 years of age or older.
Employees and their immediate family members (including spouse, parents, children, siblings, and their respective spouses) of the Contest Sponsor are not eligible to enter the Contest or to win the Prize.

When does the Contest Start and How do I Register?

Registration begins Feb 8, 2019 and ends May 31, 2019 at 11:59 p.m. (MDT); you must be registered to participate in the Contest.
The Contest begins on March 1, 2019 at 00:01 a.m. (MST) and ends May 31, 2019 at 11:59 p.m. (MDT) ("Contest Period"). That gives you 90-days to experience how PhysIQ™ recharges and balances your body's natural weight management system. You must register before the end of the Contest Period in order to be eligible.
In order to be eligible, participants must:
Submit before/after pictures as well as body measurements (including weight and body fat %) holding a date stamped newspaper in the photos as well as in the photos of the scale when measuring body weigh;

Contestant Evaluation

At the end of the contest period, all contest entries will be sorted into two (2) categories, one (1) male and one (1) female. During the applicable selection period, winners will be chosen by calculations of:
Total Body Transformation Photos (50%): The overall transformation of each participant's appearance, judged by the Contest Sponsor both from the original "before" and "after" photographs.
Body Fat Lost (20%): The total body fat lost by each participant.
Total Inches Lost (20%): The overall inches lost by each participant.
Weight Loss (10%): The total amount of weight loss by each participant.
In the event of a tie in either category, the potential winner(s) will be determined based on the final judgment from the Contest Sponsor, in their sole and absolute discretion.
As required by Canadian law, if the winner is from Canada he/she will be required to answer a skill testing question before being awarded a Prize.

All photos become the property of the Contest Sponsor.
Any missing images from participants will result in disqualification.

Prizes

Two Grand Prizes will be awarded at the close of the Contest (each is a "Prize"). One Prize will be awarded to a female, and one prize will be awarded to a male.

Any additional costs related to the Prize or incurred as a result of accepting the Prize are the sole responsibility of the winner.

The Prize must be accepted as awarded and cannot be exchanged for any other prize or transferred to a third party.
The makeover components of the Prize have no cash value.
The Prize winner must be over the age of 18.
The Prize winner and travel companion must travel on the same itinerary. Hotel room configuration is subject to availability.
The Prize winner and travel companion must have valid travel documents prior to departure or Prize will be forfeited. The Prize winner is solely responsible for obtaining and/or renewing travel documents, including passport and/or necessary visas, in order to travel. The Contest Sponsor assumes no liability for ensuring winner and/or companion are eligible/able to travel. Certain restrictions may apply to travel, and reservations are subject to availability.
No changes will be permitted to travel dates or passenger names once the travel portion of Prize has been booked.
The Contest Sponsor reserves the right to designate all transportation particulars and routes.
Hotel accommodation is for a maximum of two (2) guests in one (1) room (NOTE: winner may be required to present a valid major credit card in winner's name at the time of hotel check-in to cover any incidental expenses).
The Contest Sponsor makes no warranties regarding the safety, condition, or fitness of the Prize awarded, and will not be responsible for any loss, damage or injury suffered by a winner through the acceptance or use of the Prize or attendance at any event associated with the Prize.
The Contest Sponsor will require the Prize winner and travel companion to sign a declaration and release form in order to receive the Prize. In such a case, the winner will not be entitled to receive the Prize without signing the declaration and release.

Winner Selection and Prize Award

The selected participant will be notified by phone and email within 5 business days after the Contest ends. A maximum of three attempts will be made to contact the selected participant over three business days. If the selected participant cannot be reached within three business days or if, for any reason, the Prize cannot be awarded, another contest entry will be selected four days after the initial draw and given three days to claim the prize and so on, until the Prize has been awarded, or no Prize may be awarded at all."
The odds of winning a prize will depend upon the total number of eligible participants in each of the male and female categories.
Before being declared the confirmed Contest Prize winner, all potential winners will be required to sign and return an affidavit of eligibility and a Release of Liability Form within thirty (30) days of notice:
correctly answer a mathematical skill-testing question without mechanical or other aid (Canada residents, excluding Quebec); and
sign and return the Contest Sponsor's Consent and Release of Liability Form by June 13, 2019 which (among other things):
confirms compliance with these Rules;
acknowledges acceptance of the applicable Prize;
acknowledges that the selected participant is liable for any additional taxes payable on the Prize amount as awarded, and acknowledge that the Contest Sponsor will not reimburse the selected participant for any additional taxes payable;
releases the Contest Sponsor and each of its officers, directors, agents, representatives, successors and assigns (collectively, the "Releasees") from any and all liability in connection with this Contest, his/her participation therein and/or the awarding and use/misuse of the applicable Prize or any portion thereof; and
If a selected participant fails to correctly answer the skill-testing question; fails to return the properly executed Contest documents or the Consent and Release of Liability Form within the specified time; and/or cannot or is, for any reason unwilling or unable to, accept the Prize as awarded for any reason including inability to travel; then he/she will be disqualified (and will forfeit all rights to the applicable Prize) and the Contest Sponsor reserves the right, in its sole and absolute discretion, to select the second highest score in the applicable male or female category who is also subject to the Contest Sponsor's verification (in which case the foregoing provisions of this section shall apply to such new selected participant), or to not award a Prize at all.

Prize Conditions

The Prize will only be awarded to the eligible participant whose verifiable full name and valid email address appears on the Contest entry.
The Contest Sponsor is not liable in the event that the flight, itinerary, or any of the tours are changed, postponed, or cancelled for any reason whatsoever. The Contest Sponsor is not liable for any travel delays or cancellation whatsoever.
The Contest Sponsor reserves the right to substitute a prize of equal or greater value for the Prize.

Privacy

The Contest Sponsor respects the privacy of each individual with whom it comes into contact, either as a visitor to this website or as a participant in the Contest.
By providing personal information, Contest participants consent to receive email or telephone communications from the Contest Sponsor for the purposes of managing and administering the Contest, the awarding of the Prize and as otherwise be related to, or necessary for, the Prize.
Each Prize winner agrees to permit the Contest Sponsor to use the "before" and "after" photos that are submitted for the Contest, and each Prize winner agrees to be photographed when the prize is awarded to the winner. Each Prize winner agrees to the publication and/or reproduction of his/her name, city, and/or photographs or other likeness without further notice or compensation, in any publicity or marketing carried out by or on behalf of the Contest Sponsor in any manner whatsoever, including print or the internet.
When posting videos, photos, and testimonials about your weight loss journey on any social media, we recommend that you review the privacy statements of the social media.

General

By entering, participants agree to abide by the Contest rules and the decisions of the Contest Sponsor which are final.
Persons whose appearance or fitness or health transformation before (even if not previously discovered), during, or after winning a Contest Prize has been in any way altered by, or is due to, in whole or in part, the use of non-medically necessary plastic/cosmetic surgery, liposuction, steroids, human growth hormone, or any unnatural method of body enhancement or weight reduction of any kind are not eligible to enter and any such entries may be disqualified, in the Contest Sponsor's sole and absolute discretion.
Late or incomplete entries will be disqualified.
By accepting the Prize, the Prize winner agrees to hold the Contest Sponsor and its affiliates, parent(s), subsidiaries, agents, and anyone else involved in the development, production or handling or administration of the Contest and their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out acceptance, use or misuse of the Prize.
All entries become the property of the Contest Sponsor who assumes no responsibility for Contest irregularities including printing or typographical errors, lost, delayed, destroyed or misdirected entries. The Contest Sponsor is not responsible for any failure of the website during this Contest or for any problems or technical malfunctions of computer online systems, websites, servers, access providers, computer equipment, software or any email, online or internet entry lost due to technical problems or traffic congestion on the internet or at any website or any combination thereof, including any injury or damage to an participant's or any other person's computer or property related to or resulting from downloading any material in the Contest. Nor is the Contest Sponsor responsible for any telephone, satellite, cable, network or other equipment, electronics, software, service, transmission, connection, processing or other errors, failures, congestion, accessibility or availability issues or for other technical or human error in the receipt or processing of entries, whether online, by telephone or by mail.
Entries submitted through fraudulent means or in any way tampered with, incorrect, unclear, forged, mutilated, illegible, indecipherable or mechanically reproduced may be disqualified at the sole discretion of the Contest Sponsor.
This Contest may be modified or canceled at any time.
